This form asks for your child's full name, date of birth, and ethnicity or race, and for your name, address, preferred language, how Stars can reach you, and what you have noticed. While you fill it in, your answers stay only in your browser. They are not saved to your device, not placed in web addresses, and not sent anywhere until you choose to send the request. When you send it, the request goes over an encrypted connection to the secure request system our website provider runs for Stars, where Stars staff review it.
